Success Story: Gwendolyn Johnson
March 24, 2023Greenville Nursing and Rehabilitation is excited to share resident Gwendolyn Johnson’s Success Story!
Ms. Gwendolyn Johnson was admitted to Greenville Nursing and Rehabilitation after a knee replacement which decreased her ability to complete transfers, walk and perform her activities of daily living without significant assistance. Before her admission, Ms. Johnson lived alone, and it was essential for her to return home Independently. While at Greenville Nursing and Rehabilitation, she enjoyed socializing with fellow therapy and facility patients, and she would participate in facility activities and parties frequently.
She participated in therapy five times weekly and was always eager to work hard. To make sure she was ready and safe for discharge, an at-home evaluation was completed with Ms. Johnson. She demonstrated good safety and modified independence during her assessment; minimal recommendations were only needed. Because of her determination and hard work with skilled therapy, she was discharged home Independently. She was given a “Red Carpet” farewell when leaving the facility, which greatly excited her!
