Brenda Clark came to us in June after experiencing a cerebrovascular accident (referred to commonly as a CVA or “stroke”) which left her weakened and unable to complete her daily activities independently. The stroke also effected her ability to communicate for a period of time. Both of these losses necessitated Brenda’s need for additional therapy after her hospital discharge.

Brenda admitted to Greenville Nursing and Rehabilitation and began a schedule of physical therapy, occupational therapy, and speech therapy to recover the strength, endurance, and abilities that she enjoyed prior to her event. Upon her arrival, Brenda was experiencing weakness in her legs which made her dependent on the assistance of staff to walk and transfer. She also required moderate assistance to complete her daily self-care routine. With her hard work and the knowledge and dedication of our therapy staff, Brenda is now able to get around on her own and able to have greater independence in her daily routine.

Brenda enjoys meal time, and says that her favorite dishes are our cheeseburger and fries and our fish. For activity, she mostly enjoys playing ball with the other residents during therapy. In her free time, you will find Brenda reading big, thick books or playing solitaire on her phone. She gets into animated features, and her favorite movie is “Despicable Me” with Gru and the Minions. We are so proud of the progress you’ve made, Brenda!

Greenville Nursing & Rehab was not just excited to celebrate St. Patrick’s Day, we were also excited to celebrate Anthony Scott, aka “Tony” discharging to home.

Tony was welcomed to our community in February 2021 with several diagnoses, including COPD, Atrial Fibrillation, Tachycardia, Muscle Weakness and Hypertension. She required moderate assistance for all activities of daily living, minimum assist with transfers. She presented with an increase in weakness and decrease in activity tolerance. Tony was able to walk without assistance a distance of 25 feet.

Tony began working with our top-notch Physical and Occupational therapist, with the goal of returning home. They helped develop a personalized plan to help achieve her goals.

She worked hard and met her goals! She heads out the door able to perform her activities of daily living independently, along with ambulating 50-75 feet with her walker. No Iris luck needed here!

We are proud to highlight Jordan Vinson MBA, MSN, LNHA, Executive Director at Greenville Nursing and Rehabilitation. Jordan is an outstanding administrator with over 15 years of experience in nursing and administration. 

“I go to work everyday with a purpose. We change lives and make life better through quality care. Residents rely on us and it’s encouraging to know we can make a difference.”

Starting out as an RN in 2006, Jordan worked at a local hospital and then in a nursing home on the rehab-to-home team. She immediately loved working in long-term care! What does she most enjoy about it? 

“My favorite part is the connections that you make with the residents. In long-term care, you have a huge opportunity to connect with people, and to make a difference, and feel like you have a purpose.”  

Jordan’s passion, work ethic, and sense of purpose helped her thrive. As an RN, her career journey included roles as staff development coordinator, unit manager, and director of nursing. Over time, she became interested in the operational side and became an administrator in 2014.

“As the administrator, you really have your hand in everything that happens at the facility, every department. You’re following up with the department heads, and just making sure that the residents have the best experience possible and that they’re receiving quality care.”

Two words come to her mind when it comes to her team at Greenville Nursing Rehab: Resilience and Dedication.

“We have so many dedicated care team members that just legitimately care. I couldn’t do my job at all if it wasn’t for the team that’s here. From department heads to the care team members, nursing, dietary, housekeeping, maintenance, and therapy, they all just go above and beyond. We’re very blessed here.”

Bluegrass Consulting Group centers remain vigilant to living our Mission that creates a magical culture through the hearts of our Care Team Members. Each Bluegrass Consulting Group center prioritizes the health, safety, and well-being of all residents, Care Team members and family members within our community. This pandemic has seen unprecedented challenges for the healthcare industry, our nation, and our world. In particular, those challenges presented to Post-Acute Care, with all the changes in regulations, knowledge, and directives, the organization has sought to be in full compliance and a leader in the healthcare industry. The recent federal vaccine mandate is the most recent example of how we are asking our organization to rise to the challenge and lead the way. Through much communication, engagement, and research, we are announcing that all Bluegrass centers will embrace the requirement of the COVID-19 vaccine to keep our residents, Care Team members, and family members safe.

Effective October 1^st, 2021 – all new hires will be required to be vaccinated.

Effective November 1^st, 2021 – all Care Team Members and contracted employees will be required to be vaccinated.

The safety, security, and health of our residents and Care Team members is our number one concern, especially during this pandemic. We are assured that this requirement will provide the best approach to quality care for our communities and those residents we have the privilege to serve. We will continue to monitor updates and governmental direction regarding the mandate and will reserve the right to change this policy, if needed, to comply with all federal mandates. If you have questions or believe you qualify for a Medical or Religious exemption, you are encouraged to speak to your Human Resource Representative or Executive Director. All exemption requests will need to be submitted no later than October 15, 2021.

Our Response to COVID-19

To Our Residents and Family Members:

We know some of you may be concerned about the spread of COVID-19 (the new coronavirus) being reported in the media and how it may impact us here at the center.

Our top priority is keeping our residents and Care Team Members safe and we feel these measures are needed to ensure their safety. We understand that connecting with family members is incredibly important and there are a variety of other ways you might consider connecting with them. These may include telephone, email, text or through Skype or Facebook. Our Care Team Members are happy to help facilitate these alternative methods.

Our center is following the recommendations of the CDC on prevention steps, including following strict hand-washing procedures, and in many circumstances, wearing gowns and gloves when interacting with residents who are sick. We also are staying up-to-date with the CDC recommendations as they are updated. In addition, the center is in close contact with the local and state health department and are following their guidance.

We will notify you if any residents or Care Team Members are diagnosed with COVID-19. Should you have any questions, please feel free to contact the Executive Director.

Ensuring residents are cared for in a safe and healthy environment is our greatest responsibility. 𝗢𝘂𝗿 𝘁𝗲𝗮𝗺 𝗶𝘀 𝗽𝗿𝗼𝘂𝗱 𝘁𝗼 𝗹𝗲𝗮𝗱 𝘁𝗵𝗲 𝘄𝗮𝘆 𝗯𝘆 𝗱𝗲𝗹𝗶𝘃𝗲𝗿𝗶𝗻𝗴 𝗮𝗻 𝗶𝗻𝗱𝘂𝘀𝘁𝗿𝘆-𝗱𝗲𝗳𝗶𝗻𝗶𝗻𝗴 𝘀𝘁𝗮𝗻𝗱𝗮𝗿𝗱 𝗼𝗳 𝗹𝗼𝗻𝗴-𝘁𝗲𝗿𝗺 𝗰𝗮𝗿𝗲 𝗰𝗹𝗲𝗮𝗻𝗹𝗶𝗻𝗲𝘀𝘀. As part of the “Greenville Defense” program, we follow a scientific approach using rigorous measures and our industry’s best products. We closely monitor recommendations from The Centers for Disease Control and Prevention (CDC). Our extensive efforts include:

Weekly COVID-19 Testing: All Care Team Members are tested at a minimum of every week.

Sign-In Kiosks and Temperature Monitoring: We utilize a digital system to screen guests and Care Team Members for symptoms of acute respiratory illness (e.g., fever, cough, difficulty breathing) before entering our community.

Personal Protective Equipment: Care Team Members and residents have access to masks (surgical, N95), gloves, gowns, face shields, and hand sanitizer. Care Team Members are required to wear a mask at all times.

Recycled Air Ionizer: Air Ionizers are installed directly into air ducts, producing positive and negative ions that neutralize harmful pollutants and odors in our buildings.

Plexiglass at Check-in and Nurses Stations: Plexiglass Shields at reception and nurses stations provide a physical barrier to reduce exposure and cross-contamination.

Electrostatic Sprayers: Sprayers provide a uniform and wraparound layer of germ-killing power, providing maximum efficiency disinfecting surfaces.

Deep-Clean Sanitization: Sanitizing lowers the number of germs on surfaces or objects to a safe level.

Daily Communications with Families: Our automated system keeps families and loved ones informed on COVID19 cases.

Juanita Buskill is known to family and friends as “Nuddie”. Nuddie arrived to Greenville with a diagnosis of Degenerative Disc Disease, Osteoarthritis, muscle weakness & Cognitive Communication Deficit. She had a poor appetite and was underweight. Upon admission, Nuddie began receiving Physical, Occupational and Speech therapy. She changed her dietary requirements and changed her diet to a Regular Mechanical Soft diet with supplements twice daily. Overtime, Nuddie’s family began to see the great strides she was making. She began ambulating with a walker, communicating with others, and even gaining weight! Today, Nuddie lives in Greenville’s Nursing Personal Level of Care home, requires little assistance with her activities of daily living and has gained over 7 pounds. Great job, Nuddie!

Mr. Richardson admitted to Greenville Nursing & Rehab from the hospital with Atrial Fibrillation & Bradycardia. Prior to hospitalization, he lived at home with his beautiful wife Sarah. Along with his diagnosis, it was noted that he had had several falls and generalized weakness. Physical and Occupational Therapy began working with him and found that he was very motivated, had a pleasant disposition and a good sense of humor, which made his therapy time fun! He was surrounded on the outside with such a wonderful support system. His family members would visit with him outside through the window daily, with his great-grandchildren holding up cards of love for him to read. Mr. Richardson continued to make significant gains in his strength, endurance & balance. He improved in his gait and transfers surpassing his prior level of function! Mr. Richardson is truly a success! Upon discharge, his wife met him outside with a kiss and a ride home!

Mr. John Agnew admitted to Greenville Nursing and Rehabilitation after being hospitalized with septic shock, osteomyelitis of right foot, acute kidney injury, hypoglycemia, and was significantly underweight. After a few months of proper nutrition and close monitoring from Care Team Members and diligent time with the therapy team, John is now up and about on his own and his health has vastly improved. John’s sister, Robbie, states often “I thank God we found you all. You have been a true blessing to me and John.”

Congratulations to John on all of his successes!

Mr. Donald Moore came to Greenville Nursing and Rehabilitation after a hospital stay.  Mr. Moore was unable to complete any tasks independently; including sitting unsupported on the edge of the bed. He was also unable to ambulate or even transfer without assistance. Mr. Moore’s wife, Mary, stayed by his side throughout his hospital stay to assist with patient care in hopes to take Donald “Duck” back home with her assistance. Mr. Moore required intense therapy to safely return home with his family support. After 3 weeks of therapy and care from nursing, Donald returned home! He made such great improvements; including walking without assistance 300+ feet and transferring without assistance. Mary stated that she is so happy with the progress he has made, and everyone has been so nice while they were in the facility. Congrats Duck on your accomplishments!
